You can obtain this information from the office. It will be emailed to you prior to registration. 7. Need Parent/Guardian/Legal Custodian Signature. Cedar Rapids, IA 52404 If you do not live in the CR Community School District mail or take your CPI form to your individual school district. If you would like to keep a copy of this form for yourself you may copy it. If you are sending the copy to the school district (not the original) make sure you have copied both sides. 8. If you want your child to be able to take classes or participate in athletics that Cedar Valley does not offer, then you must dual enroll your child. Fill in the information.
